FINANICAL POLICIES

Marvel Health Concepts (MHC) is a private, direct-pay, out-of-network practice. We do not bill insurance directly, but having insurance does not mean you cannot receive care at MHC. Depending on your plan, you may be able to submit your MHC receipt or superbill for potential out-of-network reimbursement or application toward your deductible.

1. Pay MHC Directly

Payment is made directly to MHC at the time of service. Our fees are clearly provided in advance and based on the services included in your personalized care plan. We accept cash, check, credit/debit cards, and HSA/FSA payments when eligible.

2. We Don't Bill Your Insurance

MHC does not bill insurance companies directly or participate as an in-network provider. This allows us to focus on your health needs and care plan rather than insurance restrictions or coverage requirements.

3. You Can Still Use Your Insurance

If your plan includes out-of-network benefits, you may be able to submit your MHC superbill or receipt to your insurance company for possible reimbursement or application toward your deductible. Your insurance company determines whether a service is covered and how much, if anything, it will reimburse.

4. We Make Reimbursement Simple

MHC can provide a superbill or itemized receipt for you to submit to your insurance company. A standard superbill is provided at no charge once every 3 calendar months. Additional superbill requests are $5 each.

Before beginning care, we recommend asking your insurance provider:

  • Do I have out-of-network benefits?

  • Are chiropractic services covered out of network?

  • Do I have an out-of-network deductible?

  • What percentage of eligible services is reimbursed?

  • Do I need to submit a claim myself?

5. Know Your Cost Upfront

We review your fees and personalized care plan with you before care begins, so you know what to expect. Your insurance benefits and reimbursement are determined by your insurance company and are not guaranteed. Regardless of how your insurance processes a claim, payment for services provided by MHC remains your responsibility.

6. Why Doesn't MHC Bill Insurance Directly?

MHC is a private, direct-pay, out-of-network practice. This allows us to build your care around your individual health needs and goals—not insurance restrictions or reimbursement requirements.

You pay MHC directly for services received, and we review your recommended care and fees with you before care begins.

7. Can I Still Use My Insurance at MHC?

Yes. Having insurance does not mean you cannot receive care at MHC. MHC does not bill insurance directly, but if your plan includes out-of-network benefits, you may submit a superbill or itemized receipt for possible reimbursement or application toward your deductible.

You are responsible for submitting claims and communicating with your insurance company. MHC can provide appropriate documentation when applicable, but coverage and reimbursement are determined by your insurance company and are not guaranteed.

8. Why May Some Functional Health Services or Laboratory Testing Not Be Covered?

Insurance coverage depends on your specific plan, service or laboratory test, provider, medical-necessity requirements, and billing and coding rules.

Some Functional Health services and laboratory testing may not be covered under your plan. Laboratory coverage can also vary by test and insurance plan.

Before testing, contact your insurance company to confirm coverage, deductible requirements, and your potential out-of-pocket cost.

9. What Will I Pay at MHC?

You will know your expected MHC costs before beginning care. We review your recommended services, personalized care plan, and fees with you in advance.

Payment is made directly to MHC at the time of service. HSA/FSA payments may be accepted when eligible; confirm eligibility with your account administrator.

Any insurance reimbursement is separate from your payment to MHC and is determined by your insurance company.

10. Does MHC Accept Medicare or Medicaid/MO HealthNet?

MHC does not participate with Medicare or Medicaid/MO HealthNet and does not bill these programs for MHC services.

Medicare's chiropractic benefit is limited to manual manipulation of the spine to correct a spinal subluxation when applicable requirements are met. Other services provided or ordered by a chiropractor, including many diagnostic, therapeutic, and laboratory services, are not covered under the Medicare chiropractic benefit.

If you have Medicare or Medicaid, contact your plan directly regarding coverage. We are happy to explain MHC's services and fees so you can understand your options.
